First Choice will be joining the Hope 2020 Virtual Conference on November 5-7, 2020. We will be hosting a breakout session “Hot Topics in Hemophilia” with presenter Tammuella “Tami” Chrisentery-Singleton, MD, at 11am CST on November 5, 2020. Dr. Singleton will address some of the most current topics in the bleeding disorder community such as COVID, current and next generation treatment options.
During your registration for the conference, you will selection all sessions you are interested attending each day. Once you complete this process, you will receive an email with the links to match your custom schedule.
As a thank you for visiting our virtual booth, we will be hosting a drawing for a $250 Visa Gift Card on November 7, 2020. To be considered for the drawing, please fill out the information below no later than November 6, 2020 11:59:59pm CST and you will be entered. The winner will be announced the last day of the Hope Conference and contacted via email for delivery instructions.

First Choice is proud to bring you a new service to our site. We now offer Enrollment Forms available for use with your patients. On this list, there are several forms focused on various diseases and their respective medications ready for you to download, complete, and submit to the team.
You can submit the form electronically via the Submit button on each form if you are using Adobe Acrobat, fill out and email the form to referrals@firstchoiceiv.com, or fax the form to 844-324-3244. We also welcome any other form or document of your preference. These are simply aides available for your use.
Additionally, we also accept E-Prescribe submissions to First Choice Home Infusion via NCPDP 4446654 or NPI 1366854788.
